The Type 2 Diabetes Path to Remission programme is a free service that aims to help people transform their health, lose weight and put their type 2 diabetes into remission.
The programme takes place over 12 months, including a 12 week 800-850 kcal/day diet using meal replacement products such as shakes and soups. You will be supported by a diabetes clinician, or a clinician-led support group at a time that suits you. This combined with the Oviva app and online Learn platform supports you to improve your health and better self-manage your condition.
We will be monitoring your sugar levels (hba1c level) in practice and will be able to provide support – for example if medicines need to be changed.
What happens on the programme?
You will be offered total diet replacement products including soups and shakes consisting of up to 900 calories a day for up to 12 weeks. During this time you will be expected to replace all normal meals with these products.After this 12 week period you will receive help to re-introduce food in a healthy and sustainable way. You will receive support from your coach throughout the rest of the programme to embed new healthy habits and maintain your weight.

What do I need to do?
You should have received a message about the programme from the practice. If you would like to be referred, please reply to the message/email or get in touch with the practice to discuss whether you would likely benefit from the programme with one of the clinical team.
