NHS App
Try the NHS App. If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet. You can use the NHS App to get health advice, order repeat prescriptions, manage your hospital referrals, view your GP health record and more. If you already use Patient Access you can continue to use it but you can use the NHS App as well. For more information go to www.nhs.uk/nhsapp.
What the NHS App does
- Submit online request for non-urgent medical and admin queries – get advice and care from your GP surgery using and online form.
- order repeat prescriptions – see your available medicines, request a new repeat prescription and choose a pharmacy for your prescriptions to be sent to.
- book appointments – search for, book and cancel appointments at your GP surgery, and see details of your upcoming and past appointments including certain hospital appointments.
- check your symptoms – search trusted NHS information and advice on hundreds of conditions and treatments, and get instant advice or medical help near you.
- register your organ donation decision – choose to donate some or all of your organs and check your registered decision.
- Receive messages and notifications – view messages from your GP surgery and get notifications through your phone or tablet
- view your GP health record – including information about medication, allergies, vaccinations, previous illnesses and test results.

Online access forms
You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at www.nhs.uk/gponlineservices
Please download NHS app and self-verify to get GP medical record access. If for some reason, your account is unable to connect to GP record, please complete the relevant forms and bring them into the Surgery between 16:00 – 18:00 Monday to Friday, along with photo ID and proof of address (council tax, electricity, water or gas, or a letter from your landlord or Housing Department to confirm your residency).
Once forms are completed and submitted at reception, GP (Data Controller) will review the request and admin will get back to you via email or telephone regarding access. This will take roughly 14 working days.

If you’re a family member or friend, you can apply to access someone’s GP services to help them manage their care. Find out more at www.nhs.uk/apply-for-proxy. Please complete below form and submit at reception. We will need to see photo ID and proof of address of the patient and proxy user.
